To start receiving surveys, go to the Colmar Brunton sign-up page (you'll need to be signed in to My Fly Buys to do this). Once you've signed up, you'll receive surveys from time to time based on your age, gender and where you live (each survey is on a different topic, so you may not qualify for every one).
